Making a Referral Hospice exists to provide support and care to people in the last phases of an incurable disease or illness so that they may live as fully and comfortably as possible. Hospice services provided by MHP will still primarily be centered in the home. However, when a patient or family expresses a desire to no longer be at home, and the patient’s physician and hospice care team feel an inpatient setting would benefit them; a transfer to the Serenity House can be made. Examples of when a transfer to the Serenity House may be needed:
Regardless of a patient’s ability to pay, no one will be denied in-home hospice services or admittance to the Serenity House. For questions or to make a referral to the Serenity House or MHP Hospice Services, call 641-672-3260. |
