Palliative care is a short term, home based program for people who need help with pain and symptom management and other supportive care. The purpose of the program is for people who do not qualify for traditional home healthcare and are not ready for hospice services. As part of the Palliative care program, staff works with your primary care physician to provide pain management and symptom control as well as support to you and your family.



Services provided:

  • Nursing – A nurse will coordinate with your primary care provider and visit one to two times per week to assess pain and other symptoms. The nurse is on call 24 hours per day and can assist with medication management and provide individualized support.
  • Home Care Aide – May be assigned to visit up to five times a week to assist with activities of daily living.
  • Social Worker – Available to assist patients and their families as needed.
