Welcome to the new Mahaska Health MyChart®. MyChart® is the same system used by the Top 20 hospitals in the U.S. MyChart® is powered by Epic, the nations top health record management system and patient portal.

This is the same application utilized by Mayo Health System, Cleveland Clinic, Stanford, Vanderbilt, UIHC, and UnityPoint. Mahaska Health utilizes MyUnityPoint as our electronic health record vendor for the Epic patient portal system. You may come across the MyChart® name from time to time when registering for access or using a mobile device to access records and health management tools.

Enrollment Questions

There are three ways to get an account:

  1. At your next clinic appointment, your After Visit Summary will include an activation code. This code will enable you to log in and create your own username and password.
  2. Visit mahaskahealth.org/mychart-activation and follow the steps to create your account.
  3. You may also call our support team at 641.672.3200, or ask to sign up during your next office visit.

If you have technical questions about using the MyChart® app, email the technical support team at MyUnityPointSupport@unitypoint.org or call (877) 224-4430 Monday – Friday 8AM – 5PM. Please remember, the MyUnityPoint technical support team is not to be used for medical needs, medical questions or anything urgent; for medical emergencies, dial 911.

After You've Enrolled

You may click the “Forgot password” link on the sign-in page to reset your password online. You may also contact the technical support team at call (877) 224-4430 Monday – Friday 8AM – 5PM to request a new, secure password.

For security reasons, your activation code expires after 90 days and is no longer valid after the first time you use it. For activation code problems, please email the technical support team at MyUnityPointSupport@unitypoint.org, or call (877) 224-4430 Monday – Friday 8AM – 5PM. You are also able to create a Mahaska Health MyChart® account by visiting our MyChart® login page and click on the “Sign Up Now” button to begin verification.

You are also able to create a MyUnityPoint account by visiting Mahaska Health MyChart Login and click on the “Sign Up Now” button to begin verification.

We aim to protect your privacy and keep your information secure. While logged in, if your keyboard remains idle for 15 minutes or more, you will automatically be logged out. We recommend that you log out of your session if you need to leave your computer for even a short period of time.

Yes, you can also download the free MyChart® application from the iPhone App Store or Google Play by searching for “MyChart”. After downloading the MyChart® Epic® patient portal system, select ‘MyUnityPoint’, our third party electronic health record vendor from the app’s list of providers.

Appointments and Messaging

You will generally receive an answer within two to three business days. Please note that your provider may approve someone else on your care team to respond to your message. Also, please note that the MyChart® app should not be used for urgent situations. Please call us at 641.672.3360 if you have an urgent medical question or medical need, or dial 911 if it is an emergency.

You can schedule an appointment through the MyChart® app with:
• Any Primary Care Provider you have received care from in the past three years – or any member of that Provider’s Team (at that location)

If you are not able to find the provider you are looking for, please use our find a doctor tool and call the provider’s office directly for an appointment time that works for you.

If your provider, reason for your visit or specialty isn’t an option in the scheduling feature on the MyChart® app, you still have the option to request an appointment. All appointment requests submitted through this feature are sent to our scheduling team who will review your request and follow-up via message through MyChart® or phone call to confirm availability with you.

You may cancel an appointment if it is more than 24 hours from the time of your appointment. Contact your provider’s office directly by telephone if you need to cancel less than 24 hours in advance. Please follow the cancellation policies established by your provider.

Patients have the option to personalize how they would like to receive notifications regarding activity in their MyChart® account. Notifications are available by email or text message regarding appointments, test results, messages, medications, questionnaires and more.

From the web portal MyUnityPoint main menu, choose Settings > Notifications > Accept the disclaimer, and then choose your notification preferences on the Notifications Settings page.

Please ensure your notifications are set correctly for the best experience. Your MyUnityPoint notifications are separate from any additional text message reminders you may have already signed up for through your provider.

Proxy Access

Yes. Proxy access allows you to use MyChart® on behalf of someone else. Common examples of when proxy access is beneficial include a dependent child, aging parent or spouse/partner.

To request access to use MyChart® on behalf of someone else, log in to your MyChart® account and select Request Proxy Access under the Messaging menu option.

Due to complex teen privacy laws, there are certain types of medical information that the parent or guardian of a minor patient age 12 or older may not view without consent of the minor patient. Due to these requirements, we offer limited information to proxies of minor patients in this age category.

Pediatric Access: Parents and guardians of children up to 12 years of age can:

  • View upcoming appointments
  • View test results
  • View health history including immunizations, allergies, medications and more
  • Send a non-urgent, secure message to your child’s clinician’s office

Limited Teen Access: Parents and guardians of children age 12-17 can:

  • View immunizations records

Teen Connection: Patients 14-17 years of age can view their own health information. *This is not proxy access as the patient is accessing his/her own health information.

At this time, there is no way to limit the type of information an approved proxy can access. There are two options: no access or full access to your account.

You can remove a family member’s proxy access to your MyChart® account at any time by logging into your MyChart® account. Click on “Personalize” under the Settings menu option on the top right and then select the proxy access you wish to revoke under “Who Can View My Record?”

Technical Questions

MyChart® is a secure website maintained by Epic Systems Corporation that helps you conveniently organize your health information and connect multiple MyChart® accounts from multiple organizations. Mahaska Health utilizes MyUnityPoint as our electronic health record vendor for the Epic patient portal system.

MyUnityPoint is owned and operated by UnityPoint Health and is fully compliant with federal and state laws pertaining to your privacy as directed by the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by MyUnitypoint or Mahaska Health. Your medical information can only be viewed by people who are legitimately involved in your care. Clinicians and other staff operate under strict confidentiality rules.
