Mahaska Health patients can sign up for a MyChart® account after an appointment using the activation code located on your After Visit Summary. The activation code that you receive at your appointment will allow you to activate your new MyChart® account in the Epic® patient portal system.
If you have received healthcare services from a Mahaska Health physician or provider, your health information will be accessible after your next visit with us when you activate your account.
